Robust linear regression model of Ki-67 for mitotic rate in gastrointestinal stromal tumors

چکیده
Risk stratification of gastrointestinal stromal tumors (GISTs) by tumor size, lymph node and metastasis status is crucially affected by mitotic activity. To date, no studies have quantitatively compared mitotic activity in hematoxylin and eosin (H&E)-stained tissue sections with immunohistochemical markers, such as phosphohistone H3 (PHH3) and Ki-67. According to the TNM guidelines, the mitotic count on H&E sections and immunohistochemical PHH3-stained slides has been assessed per 50 high-power fields of 154 specimens of clinically documented GIST cases. The Ki-67-associated proliferation rate was evaluated on three digitalized hot spots using image analysis. The H&E-based mitotic rate was found to correlate significantly better with Ki-67-assessed proliferation activity than with PHH3-assessed proliferation activity (r=0.780; P<0.01). A linear regression model (analysis of variance; P<0.001) allowed reliable predictions of the H&E-associated mitoses based on the Ki-67 expression alone. Additionally, the Ki-67-associated proliferation revealed a higher and significant impact on the recurrence and metastasis rate of the GIST cases than by the classical H&E-based mitotic rate. The results of the present study indicated that the mitotic rate may be reliably and time-efficiently estimated by immunohistochemistry of Ki-67 using only three hot spots.

Prognostic analysis of primary extra-gastrointestinal stromal tumors
Objective: To analyze factors related to the prognosis of primary extra-gastrointestinal stromal tumors (EGISTs). Methods: The pathological and clinical data of 42 patients with primary EGISTs was collected and retrospectively analyzed. The patients were hospitalized from January 2004 to December 2013 and received complete surgical resection in Ruijin Hospital. The follow-up of postoperative pa...
متن کاملInterobserver variability of mitotic index and utility of PHH3 for risk stratification in gastrointestinal stromal tumors.
OBJECTIVES Accurate grading of gastrointestinal stromal tumors (GISTs), based on mitotic index, can be problematic. METHODS In this study, we compared interobserver variability in detecting mitosis on H&E with PHH3 immunohistochemistry (IHC). In addition, we examined the correlation between H&E mitosis and Ki-67 and the association of PHH3 and Ki-67 with overall survival. Four pathologists in...
متن کاملHistological and Immunological Evaluation of Gastrointestinal Stromal Tumors
Background: Gastrointestinal stromal tumors (GISTs) constitute the majority of gastrointestinal mesenchymal tumors. They usually express a proto-oncogen protein called CD117 detected by immunohistochemistry. This study investigated the differentiation of GISTs as well as the risk of aggressive behaviors in GISTs from surgically-treated patients in university affiliated hospitals. Methods: The c...

OBJECTIVE We aimed to review our archives in order to evaluate both the diagnostic and prognostic significance of DOG1 on gastrointestinal stromal tumors (GISTs), and add further insight about those issues to the current literature including some conflicting results. MATERIAL AND METHOD DOG1 was evaluated in 100 cases of GISTs, immunohistochemically. Immunostaining index was counted for each ...
متن کاملAnalysis of the amount of tissue sample necessary for mitotic count and Ki-67 index in gastrointestinal stromal tumor sampling.
There are no established opinions concerning whether the amount of tissue affects the accuracy of histological analyses in gastrointestinal stromal tumors (GISTs). The aim of the present study was to investigate the appropriate amount of tissue sample needed for mitotic count based on the risk classification of GISTs and the Ki-67 index using the following three methods: endoscopic ultrasound-g...
